We provide limited, one-time annual financial assistance to adults with mental or chronic diseases, who meet the following eligibility criteria:
- Applicant must provide evidence of US citizenship & legal residency in Wake County, NC (must have lived in Wake County for 6 months or longer with intent to stay).
- Applicant must be 18 years of age or older, living with a diagnosed chronic or mental health illness (of 3 months or longer), and working with a social worker, case manager/coordinator or health care provider. A self-referral is acceptable if all eligibility criteria are met and applicant is NOT working with an agency representative.
- Applicant must verify an annual income in conjunction with Federal Poverty Guidelines and prove he/she is inadequately insured or uninsured.
- Applicant must have a legitimate personal financial hardship that is preventing him/her from managing their medical needs.
Applications are reviewed on a first-come, first-serve basis as funds are available. A fully completed application MUST BE SUBMITTED by the referring Wake County agency or hospital before assistance can be determined. Please allow 2-5 business days for a response. LFWC reserves the right to deny any incomplete or falsified application.
